Case Management Plus
Case management is a collaborative way to assess, plan, facilitate and advocate services to meet a person’s holistic needs via communication and resources for high-quality cost-effective outcomes.
Rather than referring to multiple third-party service providers, case managers personally deliver as many services as possible.
Our case managers foster choice. Alone and in teams, we work in the community with:
- Activity coordinators.
- Financial advisors.
- Aged care placement consultants.
- Clerical services.
- Any party relevant to a person’s life.
One of our case management clients recently said:
‘You always get things done. I don’t see you for long, but I know I can rely on you.’
Though this lady has dementia, she knows we organise everything in her life – including in-home care.
If, for example:
- She’s unwell, we can reach her doctor.
- Her incinerator smells, we know how to fix it.
- She’s unhappy with her meals, we can sort it out.
- She doesn’t like a particular carer, we get a replacement.
This is what case management means ‘on the ground’.
It can range from a sole task to the comprehensive handling of complex lifestyle transitions and living arrangements.
We limit our caseload to ensure clients enjoy frequent contact with and 24/7 access to people they know and trust.
By collaborating with each client to develop a case management plan that reflects their service choices and preferences, we empower them by expediting their wishes.
Whether a specialist service is provided by us or others, we always monitor quality and outcomes to ensure all client needs are properly met.
