In Addition to providing safe transport to home, our staff pack their bags; check that all personal belongings are collected from drawers and cupboards and liaise with nursing staff to guarantee that any scheduled post-operative procedures required before discharge are met i.e Specialist appointments; requested X-rays or blood tests or the collection of any medications or dressings ordered from the hospital pharmacy.

At Call on Clare, our goal is to deliver the best and most appropriate service to our clients. Whether it is working with GP's, specialist doctors, hospitals, or coordinating other services, Call on Clare staff are committed to ensuring the highest of professional standards.

Call on Clare provides services which allows a person to maintain their independence and enjoy better participation in the wider community.

Call on Clare employs nurses who understand that maintaining dignity and independence is intrinsic to the holistic health of the person. Medicare Locals provide a range of primary health care services in the community, however, there are times when accessing these services are beyond the physical or financial capabilities of clients.

Call on Clare staff can assist by providing the following services;

  • Act as a resource person and advocate for the client
  • Liaise with the persons Doctor/Specialist or primary health worker
  • Conduct a physical assessment of client to ensure all health needs are addressed
  • Consult and coordinate any visiting 'in home service' ie, personal care, shopping, housekeeping,
  • Ensure adequate nutritional meals are provided
  • Arrange for allied services as needed – physiotherapy, occupational therapy,
  • Review medication administration system as required (in consultation with Pharmacist)
  • Provide appropriate documentation records
  • Liaise and communicate with nominated/appropriate family members
  • Arrange for appointments and attendance to health specialists
  • Provide transport and escort to appointments/hospice

Case Management Plus

Case management is a collaborative way to assess, plan, facilitate and advocate services to meet a person’s holistic needs via communication and resources for high-quality cost-effective outcomes.

Rather than referring to multiple third-party service providers, case managers personally deliver as many services as possible.

Social Events & Outings

Unable to drive? We can support and enable you to get around. Whether you wish to shop, catch up with friends, attend an event or see a movie or show, we can transport you in our vehicles or, if you’re wheelchair bound, accompany you in a maxi taxi.

The transport requests we handle are as diverse as our clients. We’ve helped people buy power tools, attend a Phillip Island BBQ and enjoy high tea in Warrnambool. We delight in assisting any way we can.

When surgery recovery takes longer than expected, outings avert ‘cabin fever’ by boosting morale, improving thoughts and making patients feel more positive about their recovery.

We remove the hassles of getting out and about, by combining safe transport with professional staff who know how to attend to special needs, administer medicine and deal with aids.

Clients are safe, well looked after and happy.

In addition to day trips, we also assist with longer journeys and holidays.

Past masters at catching flights, we can book tickets, transport people to the airport, organise aids and luggage, ensure they’re safely on the plane and do all else that’s necessary – even accompanying clients to their destination.

When picking up a person, we’re always early and we ensure they know who to look for by sending a staff member they recognise.

For those who live in facilities

Sharing the same walls with the same people 24/7 can drive anyone to distraction. Although facilities provide activities, these are generally not focused towards the individual person’s interests. Plus, today’s harried staff have very little time for one-on-one interaction.

To remedy this, facilities, trustees, families and clients regularly approach us to help provide social outings in sync with people’s needs, interests and lifestyles.

Socialising is energising. After just two months of using our service to attend outings, one of our clients experienced major quality of life improvements. From feeling sad, angry and frustrated and dwindling to just 36 kg, this lady blossomed. She gained weight, became more communicative, shook off her chronic depression and enjoyed her days more. Her loved ones and carers were astounded at the transformation.

Even getting out just once a month helps enormously.
